Tryout participation form

Every player who wants to try out must complete this form, which includes a medical release and a liability waiver. If the player is under 18, a parent or legal guardian must sign. It takes a couple of minutes.

Medical release & emergency authorization

As the parent or legal guardian, I authorize Mercy Athletics and its softball program Mercy Fastpitch, and their coaches and staff, to obtain emergency medical care for my child if I cannot be reached, including emergency transportation by ambulance, treatment by an emergency physician, admission to a hospital if medically necessary, and the administration of medications by licensed medical providers. I certify that my child is physically capable of participating in softball activities.

Assumption of risk & liability waiver

I understand that participation in fastpitch softball involves inherent risks, including but not limited to being struck by bats or balls, sliding and collision injuries, falls, heat illness, and in rare cases serious injury or death. To the fullest extent permitted by Washington law, I release, waive, and hold harmless Mercy Athletics and Mercy Fastpitch, their coaches, staff, and volunteers from any claims, damages, attorney fees, or liabilities arising from my child's participation, except where prohibited by Washington law, to include claims arising out of ordinary negligence related to participation. This release does not apply to gross negligence or intentional misconduct.

Photo & media release policy

I authorize Mercy Athletics and Mercy Fastpitch to photograph and video my child and to use those images on its website, social media (Facebook, Instagram), recruiting and promotional materials, and printed publications.
